Utility Applications

All applications for new utility service must be completed online - CLICK HERE TO BEGIN THE ONLINE PROCESS.

*If you are unable to complete the online process, you may visit the City Administration Building and the online process will be completed for you.

To set up automatic payments using a checking or savings account, download and complete the ACH Authorization Form below, then submit the completed form and a copy of a voided check to the Utility Billing & Collections Office.

Completed forms and voided checks can be submitted by:

  • Delivering the completed form and voided check to the City Administration Building
  • Faxing the completed form and voided check to (513) 360-2232
  • Mailing the completed form and voided check to:

  • For security purposes, please do not send your completed form and voided check via email.
